Building Inspector/Assessor Joseph Sperber Fire Inspector Peter Anfiteatro MEMORANDUM Date: February 13, 2012 To: Abe Zambrano From: Daniel O’Connor Subject: Sidewalk and Curb Improvement Program Contract Change Order #2 (Duck Pond Park Drainage Work along new Sidewalk) The Village entered into a contract with Paladino Concrete Creations Corp. on 6/7/10 for the Sidewalk and Curb Improvement Program. This contract was extended by mutual agreement until 6/7/12. At the request of the Village the contractor performed sidewalk and curb installation work on Bungalow Road between the two parking lots in the Duck Pond Park. While working on the sidewalk and curb on Bungalow Road the contractor was requested to perform additional work not included in the base contract. This additional work consisted of drainage improvements that were necessary to handle storm water runoff from the Duck Pond Park. The drainage improvements collect the storm water before it crosses the sidewalk, reducing impacts to the sidewalk and possible surface water ponding conditions in the park. Yard drains and drainage pipes were installed and connected to the existing storm water drainage system on Bungalow Road. This work was previously contemplated in the 2008 Park Improvement Plan that was submitted with a grant application for park improvements; the grant application however was not approved by the State. In 2008 the Village had hired a landscape architect to prepare the grant application and bid documents if the grant was approved. Purchase order #00900507-00 was issued in the amount of $20,000; the balance of the PO was $16,456.90. The PO was recently closed and the balance of $16,456.90 was returned to the Duck Pond capital account (#H7140-2103-09196) bringing the balance to $23,658.49. Change order #2 for Paladino Concrete Creations will be charged to this capital account leaving a balance of $6274.70. The balance is slated to be used for the following at this time: removal of a few dangerous trees near the play ground, trash rack over drainage inlet structure behind home plate to reduce flooding and erosion of the ball field, mechanical skimming of the pond to remove aquatic plants, and storm water flyers and magnets for Phase 2 Education and Outreach program in the Duck Pond watershed, these are being designed by CHHS AP Environment Studies students and other students. Sidewalk and Curb Improvement Program Contract Change Order #2 (Duck Pond Park Drainage Work along new Sidewalk) Please place this on the next Village Board agenda if required for the Village Board to approve the change order #2 in the amount of $17,383.79.
