S E ARCH GR OUP Rate Sheet (Attachment A) = Croton-Harmon Union Free School District: Effective July 1, 2026, charges will be based on the following hourly rate schedule: Hourly Rate $175/Hour Medical Director Consulting — en Nurse Practitioner vi7s/Hour $87.50/Hour $67.50/Hour $45/Hour Speech Pathologist $85/Half-Hour Physical Therapist $85/Half-Hour Occupational Therapist $85/Half-Hour SLP Evaluation $450/Evaluation PT Evaluation $450/Evaluation OT Evaluation $450/Evaluation * Evaluation write-up is a flat fee of $450, the testing prior to the write-up will be billed at the hourly rate. Half-hour lunchtime will be paid unless the personnel is permitted to leave their duties to take a non-working unpaid break. Overtime/Holiday’s: Overtime rates are charged for all hours worked in excess of forty (40) hours in a week or according to applicable state law. Overtime must have facility supervisory approval. The overtime rate is one and one-half (1 %) times the regular billing rate for such hours. Holiday rates are charged for all nationally recognized Federal Holiday’s. The Holiday rate is one and one-half (1 %) times the regular billing rate for such hours. Minimum Requirement: In the event that a professional works less than six (6) hours in a scheduled shift, the facility will be billed for six (6) hours as a minimum requirement. (Only applies to RN/LPN/CNA) ***For a Nurse Practitioner the minimum is four (4) hours. Medical Director Consulting: Our Nurse Practitioner will consult as the Medical Director on a weekly basis. We will bill 2 hours per week for our Nurse Practitioner to be on call. If our Nurse Practitioner works more then 2 hours per week then the School will be billed the additional time worked during that week. Late Call/Rapid Response: In the event of a late call for service (late call defined as within 24 hours of the beginning of the scheduled shift), PearlCare will bill the school for the entire shift as long as the Health Care Professional can get to the School at an acceptable time as agreed upon by the school and PearlCare at the time of the call. All hours for this shift will be billed at a premium rate which is defined as 1.5 multiplied by the hourly rate for said professional. Cancellation: PeariCare will bill client for 4 hours of service if a scheduled shift is cancelled within 24 hours of the start of the shift S EAR CH G ROU P One to One Assignment Cancellation: Pear|Care will bill client for the entire shift for each day the student (who has a one to one Healthcare Professional (RN, LPN, CNA) assigned to them) is not in school. School will use said professional in the office or another assignment as they see fit. Placement Fee. SCHOOL will take no steps to recruit or hire as its own employees those Personnel provided by PEARLCARE during the term of this Agreement. SCHOOL understands PEARLCARE in not an employment agency and that Personnel are assigned to the SCHOOL to render temporary service(s) and are not assigned to become employed by the SCHOOL. The SCHOOL further acknowledges the substantial investment in business related costs incurred by PEARLCARE to include advertisement, recruitment, interviewing, evaluation, reference checks, training, and supervising Personnel. In the event that SCHOOL, or any affiliate, subsidiary, department, or division of SCHOOL hires Personnel, SCHOOL will be in breach of this agreement and SCHOOL agrees that damages would be too difficult to calculate. Accordingly, SCHOOL agrees that if SCHOOL recruits or hires Personnel who has been introduced to SCHOOL through this Agreement, SCHOOL agrees to give PEARLCARE one hundred eighty (180) days notice of its intent to hire, continuing to staff Personnel through PEARLCARE for a minimum of thirty- six (36) hours per week through the one hundred eighty (180) days notice period or, SCHOOL agrees to pay PEARLCARE liquidated damages of thirty thousand dollars ($30,000.00).
