Dear Mr. Balbi:
As requested, we respectfully submit the following proposal for work associated with the following
scope of work, at the above referenced project.
Scope:
•
Code 753
•
Mobilization/Demobilization
•
Furnish all labor and equipment to mill 2” of asphalt pavement, inclusive of key cuts, sweeping,
and offsite disposal of millings
•
Furnish all labor, materials, and equipment to install 2” of 12.5 F3 asphalt top, inclusive of
compaction testing
•
Furnish and install asphalt tack at a rate of .05 gallons per square yard
•
Furnish and install asphalt joint adhesive
•
MPT for our work consisting of temporary construction signs, cones, and flagmen
Our lump sum change order request is based on contract unit pricing as per the attached
breakdown is : Seventy-Five Thousand Thirty-Six Dollars and Fifty-Five Cents ($75,036.55).
Sales tax is not included and will be added unless a tax-exempt certificate is provided.
Qualifications:

Asphalt price adjustment applies
•
Our proposal is based on approximately 3067 SY
•
Pavement markings are excluded
Exclusions:
•
Any work not listed above
•
Nights, weekend, overtime, or holiday work
•
Police for MPT
•
Design, Survey, as-built survey, bonds, and/or special insurance requirements
•
Testing or inspection unless noted above
•
Permits and associated fees

Temporary facilities or controls
•
Geotechnical engineer, electrician or plumber
•
Testing, handling, treating and removal of any contaminated/hazardous materials and the
installation/removal of any materials not specified above

Rock removal
•
Building excavation, underpinning, or supply of any inserts
