Scope of Work
Cured-in-place lining of two sections of 36-inch CMP on Scenic Drive in Croton-on-the-Hudson as follows. The upper section
is 117 ft. long and the lower section is 140 ft. jong. The CIPP lining will need to be completed in two shots due to middle
manhole being a drop manhole.

Proposal Inclusions
e Standard insurance certificates
e =6Allrequired safety equipment to complete the work.
e Cleaning and TV inspection of 36-inch storm line, PACP certification.
e ~All CIPP work will be completed per ASTM F1216.
e Pre & post CCTV inspection captured digitally supplied to customer via link or zip drive, etc.
© Bypass pumping which includes a 4-inch pump and soft bypass hose.
e = Traffic Control Consisting of a Simple Sign Package & MDC type Traffic Cones.
e = Allsafety equipment for confined space entry, including meter, tripod, and blower.
e Work will need to be coordinated during a non-weather event.
Proposal Exclusions
¢ Special insurance, permitting, bonding.
e Water necessary to complete the work within reasonable proximity to the work from a hydrant connection.
¢ Disposal of debris removed from storm line during the cleaning operation.
e Access to all manholes within 35 feet by our equipment is assumed.
e Bypass pumping more than outlined above.

e = Excavation of any kind
® Removal and replacement of fencing and restoration of landscaping
e Town to provide skid steer with forks or mini excavator to assist with removing liner from truck for lining
the section downstream section.

Special Conditions
° While every reasonable effort to complete the work described above will be made, unknown field
conditions and the unique nature of this task may prevent completion of the project. Furthermore, if any
of our equipment becomes lodged or stuck in the pipe for any reason, and we are unable to free it, it will
become the responsibility of the client to repair such collapsed, broken, or defective pipe and retrieve our
equipment.
e Any lateral connection that is not definitively capped shall be reinstated unless an authorized
representative of the client directs our crew in writing to open it.
@ GMPS will take no liability for any damage (i.e. nicks, cuts, gouges, or punctures) made to any existing
PVC, or like material, lateral connections during the lateral reinstatement process.
e Green Mountain Pipeline Services, LLC will take no liability for any blown toilets, sinks, tubs, etc. that may
occur from cleaning the pipes if industry standard equipment and best practices are used and GMPS does
not approach the work in a reckless manner.
Indemnity
¢ To the fullest extent permitted by law, Client shall indemnify, hold harmless and, if Green Mountain
Pipeline Services, LLC (Contractor) so opts, defend Contractor, and each of its officers, directors,
members, employees, agents, affiliates, representatives, subsidiaries and partners (each an “Indemnitee”)
from and against ali claims, damages, judgements, settlements, losses and expenses relating to or arising
out of this contract.
