Please complete Section A or B and return this form within thirty (30) days. If a response is not provided, our office will continue to process applications based on the income level previously provided by your office as cited above. For your convenience, a self-addressed envelope is included with this correspondence. If you change the maximum income level after this time frame, please notify us as soon as possible and attach the related resolution so we can update our records.
