We plan to use CHUFSD facilities for upcoming events, but we do not have a specific event at this time.
I am try to get CCoA registered as an approved organization, with a fee waiver. I did submit an application
on the “ML Schedules” website, and I was advised to submit this form. Please let me know you want anything
else. I have submitted by insureace certificate and proof of CCoA’
s 501(c)(3) status in NYS.
