--- PAGE 1 --- 2025-2026 CONTRACT FOR HEALTH AND WELFARE SERVICES This agreement made in duplicate this March 13, 2026 by and between the Board of Education, City School District of New Rochelle, New York, and the Board of Education of the Croton-Harmon U.F.S.D., New York. WITNESSETH, that whereas the School District of Residence is duly empowered by the provisions of Section 912 of the Education Law to enter into a contract with the School District of Location for the purposes of providing health and welfare services to the children residing in said school district and attending non-public schools in the City School District of New Rochelle to begin on September 3, 2025 and to end on June 26, 2026. NOW, therefore, the School District of Residence agrees to pay the City School District of New Rochelle the sum of $1644.00 per pupil for health and welfare services provided to No. of child(ren) residing in the District of Residence and attending non-public schools in the City School District of New Rochelle. It is further agreed that at the end of the current 2025 - 2026 school year the foregoing charge will be recomputed using actual expenditures for health and welfare services and actual enrollment as of October 1, and that a refund or additional charge will be made as required based on said re- computation. And the City School District of New Rochelle hereby agrees with the Croton-Harmon U.F.S.D.as follows: 1. That the following services to be provided shall include, but not be limited to, services performed by a physician, dentist, dental hygienist, nurse, school psychologist, school social worker, and school speech correctionist; taking of medical histories; vision and hearing tests; examination for employment certificates; notification of parents regarding defects and follow-up; instructions for first aid care for school emergencies. 2. The City School District of New Rochelle will also furnish the following to be used in providing such services as requested by the non-public school authorities: Supplies and equipment for use by physician, school nurse, psychologist, dental hygienist, social worker, and speech correctionist (i.e., Scales, Vision and hearing testing devices, Health record forms, First-aid supplies and all other readily transportable equipment and supplies pertaining to delivery of services). It is expressly agreed by and between the parties hereto that the services agreed to be supplied under this contract shall not include any teaching services.
