--- PAGE 1 --- CONTRACT FOR HEALTH and WELFARE SERVICE THIS AGREEMENT made in duplicate this 13 day of February 2026 by and between the Board of Education of Croton Harmon UFSD (the School District of Residence), County of Westchester of the City of White Plains (White Plains City School District), County of Westchester. WITNESSETH, that whereas the School District of Residence is duly empowered by the provisions of Section 912 of the Education Law to enter into a contract for the purposes of providing health and welfare services to the children residing in said school district and attending nonpublic schools in the White Plains City School District to begin September 4 2025 and to end on June 30, 2026 Now, therefore, the School District of Residence agrees to pay the White Plains City School District the estimated sum of $1357.68 per pupil for health and welfare services provided to approximately 6 children residing in the District of Residence and attending nonpublic schools in the White Plains City School District. It is further agreed that at the end of the 25-26 _ school year, the foregoing charge will be recomputed using actual expenditures for health and welfare service and actual enrollment as of October 15,2025 andthatarefundor additional charge will be made as required on the basis of said recomputation. And the White Plains City School District hereby agrees with the School District of Residence as follows: 1, That the services to be provided shall include, as needed, the following: services performed by a physician, nurse, school psychologist, school social worker of school speech correctionist, and may also include vision and hearing tests, the taking of medical histories and the administration of health screening tests, the maintenance of cumulative health records and the administration of emergency care programs for ill or injured pupils. 2. That the White Plains City School District will also furnish the following equipment to be used in providing such services as requested by the authorities in charge of the nonpublic school: Vision and hearing testing devices, health record forms and first aid supplies. It is expressly agreed by and between the parties hereto that the service agreed to be supplies under this contract shall not include any teaching services.
